Damaged Product Report 

If you received damaged product, or product that is non-distributable for other reasons, please follow the following procedures.

1. Keep the box that contained the damaged product.  Put damaged product aside until you have talked to the Product Recovery Manager.
2. Call Xavier Perez, Product Recovery Manager, at 512-282-2111 x153 as soon as possible.
3. Complete the form below, or fax the Product Integrity Form.  Include the date on the salvage label that the damaged product came in.
